Clinical History:
This is a 43 year old male with chronic headaches in the occipital region with nausea, visual disturbances, tinnitus, and vertigo.
Diagnosis:
Cavernous malformation or a "Cavernoma."
MR Technique:
3T (Siemens Verio). Image 1: Axial non-contrast T2-weighted Turbo Spin Echo (TR/TE 6000/96 ms, slice thickness 4 mm, scan time 1 min 12 sec); Image 2: Axial non-contrast T1-weighted Gradient Echo (TR/TE 250/2.5 ms, slice thickness 4 mm, scan time 2 min 8 sec,); Image 3: Axial T1-weighted Gradient Echo (TR/TE 250/2.5 ms, slice thickness 4 mm, scan time 2 min 8 sec) after injection of 0.1 mmol/kg of gadolinium chelate; Image 4: Axial Diffusion-Weighted Image (TR/TE 7900/100 ms, slice thickness 4 mm, scan time 2 min 7 sec).
Imaging Findings:
A 1.3 cm heterogeneous lesion with a "popcorn-ball" appearance is noted in the right cerebellar hemisphere with a hypointense rim on T1 and T2 images. Blooming artifact is noted on the diffusion weighted images. After contrast administration, no enhancement is identified.
